  Our Life Care Plan service, for clients with chronic health-care needs, is different from other legal services.

If you have chronic health problems or are caring for a parent or spouse with chronic health problems, you may wonder how you can continue to get and provide good care services.
 
 

You may feel overwhelmed by any or all of the following questions:
• How can I get the care I need, at home or in another good quality facility?
• What public benefits (including Medicaid) am I entitled to, and how can I qualify for them?
• How will I make sure my spouse continues to have a quality life if I can no longer care for myself?
• How can I assure the right financial and health care decisions will be made for me if I become unable to make them for myself?
• Do I have to spend all my assets on a nursing home, or can I preserve assets for my spouse or children?
• How do I provide for family members with special needs?

Our Life Care Plan service provides the answers and positive solutions for these and many other problems that burden you or your family.

We help our clients find, get and pay for good long-term care.
Our Life Care Plan approach is focused on achieving your good health, safety and well-being as you grow older, with three primary goals:
• We help make sure you or your loved one gets good care, whether at home or outside the home in an assisted living or nursing home setting.
• We help you, through our legal advocacy and experienced support staff, make the right decisions pertaining to your long term care and family needs.
• We help you navigate the maze of choices and options and find the best personal solutions to protect and preserve your hard-earned assets and find additional resources available to pay for quality care.

To reach these goals, we have an experienced geriatric care manager on our staff to help you every step of the way. The Life Care Plan includes comprehensive elder law, estate planning, and care assessment and coordination services to develop and carry out your personalized plan of action.

For a one-time fixed fee, we provide complete planning and follow-up services for a lifetime of care.
The one-time fixed fee includes all documents, consultations and other services (including care coordination services) related to: estate planning and asset protection; incapacity and decision-making planning; Medicaid and other public benefits planning (Veterans’ benefits, housing, etc.); client and family needs assessments; home care and assisted living/nursing facility selection; identification and application for other care resources; and on-going support from our care management staff. No hourly piece-meal charges for work due to changing circumstances. The fee for each Life Care Plan is based on whether the client is single or married, whether the client is imminently entering a nursing home or planning ahead, and on the complexity of the client’s particular circumstances.
